Dropping to Paper

If the medical certifier does not have the capability to enter the medical information electronically and the funeral director does, then the funeral director will start the case, and once personal affirmation has occurred, will select the link to “drop to paper”.  Once the link has been selected, a certificate will be printed containing only the personal information.  The funeral director will present the certificate to the medical certifier to complete the medical information manually.  Once the certificate has been completed fully by the medical certifier, the funeral director will forward the certificate to the Vital Records Unit (address provided below) to be registered by the System Administrator.  If the funeral director is faxing the completed drop to paper record, the original must still be mailed.  The death certificate will become available to the municipalities of death and residence once the death has been registered and assigned a state file number.  Certified copies may be issued at this time by these municipalities, as well as the State Office.  The death certificate will become available for viewing and producing a “working copy” for the medical certifier and funeral director once registered at State level.
